Palliative Care Program
What is Palliative Care?
Palliative Care is a unique form of health care that meets the special needs of the terminally ill. This Specialty Care helps the resident and his/her family when physical recovery from an illness or injury is no longer likely.
The goal of the Palliative Care program is to enhance the quality of life and, in some cases, prepare for the resident to return home or to another facility. Caregiver training will be provided if the patient is returning home.
Overview
Through effective pain control, psychosocial support, caregiver training, and symptom control, the program focuses on making the remaining days as comfortable and as peaceful as possible.
Using medication and other methods, a compassionate interdisciplinary team develops an individualized treatment plan. The interdisciplinary team consists of physicians, social workers, psychologists, psychiatrists, pharmacists, nurses, and spiritual counselors.

Program Evaluation
At the beginning of each treatment plan, medical personnel work closely with family members to establish individual discharge goals. Then all treatment plans are designed, monitored, and modified to meet the predetermined goals.
